Clinical Social Worker I/II
Citywide Focus

Citywide Focus is the largest Intensive Case Management program in San Francisco. We utilize an interdisciplinary team-based approach to working with people diagnosed with severe mental illness who have not been able to reach goals and stabilize at other levels of care. We are looking for Clinical Social Workers who celebrate the notion that each of us is embedded in a culture and community that provides support. We consist of four Focus team that specialize in providing culturally and linguistically competent services to the diverse communities of San Francisco.  The Cross Currents team focuses on Women, Gay/Lesbian/Bisexual/Transgender and HIV positive clients. The Hong Ling team has a focus on Asian clients. The Kujichagulia team focuses on African American and European American clients. The Senderos team specializes in working with Latinx and Korean speaking clients.

Citywide Forensics

Citywide Forensics provides intensive long-term community-based mental health services for adults with severe mental illness who cycle in and out of the criminal justice system. Our clinicians specialize in clinical case management using a “whatever it takes” approach to engage those who have historically had difficulty linking with outpatient services. Our ideal candidate would be an independent self-starter with a passion for working with underserved populations and who seeks to broaden their knowledge of providing care at the intersection of mental health and criminal justice. We are a multi-disciplinary team with master’s level or equivalent clinicians, ideal for candidates collecting hours toward licensure. Our connection to UCSF, SFGH and DPH affords us the unique opportunity to access various educational resources including publications, trainings and grand rounds.

Citywide Stabilization

The Citywide Stabilization Program serves adults with complex behavioral health needs requiring the most intensive level of outpatient response in the San Francisco system of care. Stabilization includes four teams: Linkage- working with voluntary clients in a 1:13 ratio to reduce inpatient psychiatric hospital recidivism; Assisted Outpatient Treatment (AOT)- working with clients in a 1:10 ratio who have referred by family or providers because they have historically refused help and are in danger in the community; Community Response Team (CCRT)- working with clients in a 10:1 ratio serving homeless individuals prioritized by Behavioral Health Services; and the PES Linkage Team- based out of Psychiatric Emergency Services helping develop discharge plans to reduce recidivism and providing short term linkage case management to a small caseload of clients. All teams work closely with hospitals, street-based behavioral teams, and justice-involved programs. In this role you would you develop expertise in systems navigation, crisis response, and housing resources. Our ideal candidate desires to work on a multi-disciplinary team and spend about 60% of their time in the field (except for the PES Team) doing outreaches responding to client needs

As a Clinical Social Worker in the Citywide Supportive Housing Programs, you will be supporting San Franciscans in getting into housing and maintaining their housing.  You will work with a diverse client population, and you will get to implement your clinical training and Harm Reduction skills.
