Citywide Stabilization

 

Formed in 2019, the Citywide Stabilization Program provides the highest level of care in the community for adults with severe mental illness who are harder to engage in care than most. Citywide Stabilization encompasses three programs that are designed to be short-term, engage clients immediately upon referral, and have lower caseloads (10-13 clients) to provide intensive support to clients who may need extensive outreach or have ambivalence about engaging in care. These three teams include: Citywide Stabilization Team, Citywide Assisted Outpatient Treatment (AOT) and CARE Court Team , and the Citywide PES Linkage Team.

Citywide Stabilization Team

The Citywide Stabilization Team works with clients who are referred through the Office of Coordinated Care (OCC) and meet the Stabilization level of care for short term intensive case management services.

Citywide Assisted Outpatient Treatment (AOT) and CARE Court Team 

The AOT and CARE Court Team works with clients referred by DPH AOT or DPH CARE Court for intensive case management services to be provided for the duration of their participation in these civil courts.

Citywide PES Linkage Team

The PES Linkage Team provides discharge coordination for clients in the Zuckerberg San Francisco General Hospital Psychiatric Emergency Services (PES) and offers short term intensive case management for clients who frequently present to PES and are not linked to care.
